Job summary

FOX Rehabilitation in Hamilton, NJ is seeking a licensed Physical Therapist Assistant to provide in-home, one-on-one care under a supervising Physical Therapist. Join an interdisciplinary team delivering personalized treatment plans and guiding older adults through exercises to improve strength, mobility, and confidence within FOX's Geriatric House Calls model.

You will document visits in FOX's EMR, coordinate with caregivers and community partners, and participate in mentorship and career

Qualifications

• Licensed PTA with current NJ license or eligibility.
• Graduate of an accredited PTA program.
• Current CPR certification.
• Basic computer skills including MS Office.

Responsibilities

• Deliver individualized treatment plans with supervising PT.
• Instruct patients through therapeutic exercises and functional activities.
• Document visits accurately in FOX EMR per standards.
• Coordinate care with POAs and referral sources.
