Job summary

FOX Rehabilitation, a clinician-operated practice, is seeking licensed Physical Therapist Assistants for in-home and senior living care in New Jersey. Join an interdisciplinary team with PT supervision, mentorship, and growth opportunities as you deliver individualized therapeutic activities in patients' homes and communities.

As part of the Geriatric House Calls model, you'll build lasting patient relationships, work with PTs, OTs, SLPs, and Fitness Specialists, and expand your clinical skills

Qualifications

• Valid Physical Therapist Assistant license in the state of practice or eligible to obtain.
• Graduate of an accredited Physical Therapist Assistant program.
• Current CPR certification.
• Basic computer skills; proficient in Microsoft Office.

Responsibilities

• Deliver individualized treatment plans established by your supervising Physical Therapist; adjust activities based on patient progress.
• Instruct, motivate, and safeguard patients through therapeutic exercises and functional activities; monitor progress.
• Document visits accurately and on time in FOX's EMR system in line with documentation standards.
• Stay connected with patients, POAs, and referral sources; coordinate coverage when needed.
