Job summary
FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes and senior living communities across Pottsville and surrounding areas. You’ll evaluate patients, create individualized care plans, and provide evidence-based therapy to improve mobility and independence.
The role offers autonomy with a flexible treatment schedule, possible telehealth components, and a collaborative, interdisciplinary team focused on meaningful outcomes for patients
Qualifications
- Valid Physical Therapy license in PA or eligibility to obtain.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
Responsibilities
- Provide one-on-one physical therapy to older adults in homes or senior living settings.
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions that support wellness, mobility, and independence.
- Document effectively within FOX’s EMR system per regulatory standards.
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients, caregivers, and staff to promote safety and aging in place.
- Provide telehealth/virtual care when appropriate to extend access.
