Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care in patients' homes and senior living communities in Columbia, SC. You will evaluate, plan, and deliver evidence-based therapy while maintaining thorough documentation in FOX's EMR system.

The role offers autonomy, flexible scheduling, and opportunities to incorporate telehealth as needed, supporting continuity of care and patient outcomes.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, and independence.
• Document effectively within FOX's EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
