Job summary

FOX Rehabilitation is seeking Physical Therapists to join our team in Downtown Columbia, SC, and surrounding areas. You’ll provide one-on-one PT services in homes and senior living communities, conducting evaluations and creating personalized treatment plans that improve mobility and independence.

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility to obtain
• Doctor of Physical Therapy (DPT) degree from an accredited program
• Current CPR certification
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions supporting wellness, mobility, and functional independence
• Document accurately in FOX’s EMR per regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote aging in place and safety
• Provide telehealth/virtual care when appropriate to extend access
