Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one care to older adults in private residences and senior living communities in Hilton Head, SC. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes within the Geriatric House Calls model.

Clinicians may incorporate telehealth, collaborate with an interdisciplinary team, document in FOX’s EMR, and participate in an in-home care culture that supports independence and longevity.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc.).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
