Job summary

FOX Rehabilitation is hiring a Physical Therapist in Bayside, NY to deliver one-on-one care to older adults in homes and Senior Living Communities. You’ll evaluate, plan, and provide evidence-based therapy, while coordinating with an interdisciplinary team and using telehealth when suitable.

Role offers autonomy, flexible scheduling, and the chance to impact mobility, independence, and quality of life for patients living in their communities.

Qualifications

• Licensed Physical Therapist with eligibility to practice in NY.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or living communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions supporting wellness, mobility, independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
