Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one, in-home care for older adults in the Greater Boca Raton area. You’ll evaluate, plan, and execute evidence-based therapies, while coordinating with a multidisciplinary team to support mobility, independence, and quality of life.

The role offers autonomy with a flexible schedule across in-home and senior living settings, and opportunities to incorporate telehealth to extend care access and continuity.

Qualifications

• Must hold a valid Physical Therapy license in the practicing state(s).
• Doctor of Physical Therapy (DPT) from an accredited program.
• Basic computer and EMR proficiency.
• Current CPR certification is required.

Responsibilities

• Provide one-on-one PT to older adults in-home or in senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions focusing on wellness, mobility, and independence.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote aging in place and safety.
• Provide telehealth or virtual care when appropriate.
