Job summary

FOX Rehabilitation in Oceanside, NY is seeking licensed Physical Therapists to deliver one-on-one care to older adults in homes and senior living settings. You will evaluate patients, create personalized care plans, and provide evidence-based therapy tailored to mobility and independence.

The role offers autonomy, flexible scheduling, and the opportunity to build lasting patient relationships, with optional telehealth to extend care and access for more patients.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or communities.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and quality of life.
• Document in FOX’s EMR system in accordance with standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
