Job summary

Fox Rehabilitation in New York (Nanuet and surrounding areas) is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or senior living communities. You will evaluate, plan, and deliver evidence-based PT with autonomy to manage your schedule.

Clinicians may incorporate telehealth and other virtual care to extend access, improve continuity of care, and support more patients.

Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and wellness.
• Document effectively in the EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
