Job summary

FOX Rehabilitation in Oceanside, NY seeks licensed Physical Therapists to deliver one-on-one care in homes and senior living communities. You'll assess mobility, develop care plans, and provide evidence-based treatment within our Geriatric House Calls model.

Clinicians enjoy scheduling autonomy, potential telehealth options, and meaningful outcomes while collaborating with an interdisciplinary team.

Qualifications

• Licensed Physical Therapist in NY or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one PT to older adults in homes or communities.
• Perform evaluations, develop care plans, and deliver treatment.
• Deliver therapy sessions to support mobility and independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
