Job summary

FOX Rehabilitation is expanding in San Antonio, TX, and seeks licensed Physical Therapists to deliver one-on-one care in patients’ homes or senior living communities. You’ll conduct evaluations, create plans of care, and deliver evidence-based therapy that promotes mobility, independence, and quality of life under the Geriatric House Calls model.

Clinicians may work in private residences or senior living communities, with the option to incorporate telehealth for greater flexibility.

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions to improve mobility, independence and safety.
• Document via EMR in accordance with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
