Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist for Tucson, AZ to deliver one-on-one geriatric care in patients' homes or senior living communities. You’ll design individualized plans, perform evaluations, and use our Geriatric House Calls model to improve mobility and independence.

The role offers autonomy, flexible scheduling, and the opportunity to incorporate telehealth, collaborating with PTs, OTs, SLPs, and Fitness Specialists to maximize patient outcomes and community impact.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligible to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (Private residences, Senior Living Communities, Independent Living, Assisted Living, Memory Care)
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support patient wellness, longevity, mobility, and functional independence
• Document effectively within FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth / virtual care when appropriate to extend access and flexibility

Education

DPT/Physical Therapy degree

PT license in state or eligibility

CPR certification

EMR proficiency
