Job summary
FOX Rehabilitation is seeking a licensed Physical Therapist to join our Cypress, TX team delivering one-on-one care to older adults in private residences and senior living communities through our Geriatric House Calls model.
You’ll evaluate, create individualized plans of care, and deliver evidence-based therapy with scheduling autonomy, including telehealth options to expand access. A Texas PT license, EMR proficiency, and CPR certification are required.
Qualifications
- Licensed Physical Therapist in Texas or eligible to obtain.
- Degree from an accredited PT program.
- Basic computer and EMR proficiency.
- Current CPR certification.
Responsibilities
- Provide one-on-one physical therapy to older adults in homes or community settings.
- Perform evaluations, develop individualized plans of care.
- Deliver therapy sessions to improve mobility and independence.
- Document care in FOX EMR per regulatory standards.
- Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
- Educate patients, caregivers and staff to promote safety and aging in place.
- Provide telehealth/virtual care when appropriate to extend access.
