Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one in-home and senior living care in Saline, MI. You will evaluate, design Plan of Care, and deliver evidence-based therapy to older adults, enhancing mobility and independence within patients’ homes and communities.

You’ll work with an interdisciplinary team, may use telehealth to augment care, and have schedule flexibility while making meaningful clinical impact in the communities served.

Qualifications

• Must hold a valid PT license or be eligible to obtain one.
• Degree from an accredited PT program.
• Proficient in EMR and basic computer skills.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility and independence.
• Document effectively within FOX’s EMR system in line with regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.

Education

Degree from an accredited Physical Therapy program

Valid Physical Therapy license in the state(s) of practice or eligibility to obtain

Current CPR certification
