Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to join our Detroit, MI area team providing one-on-one care in homes and senior living settings. You’ll evaluate, plan, and treat patients using our geriatric-focused Geriatric House Calls model, with autonomy to manage your schedule and participate in telehealth as needed.

Ideal candidates are licensed PTs, able to work independently, collaborate with an interdisciplinary team, and document efficiently.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
