FOX Rehabilitation is seeking licensed Physical Therapists for in-home and senior living care in Clinton Township, MI. You’ll deliver one-on-one therapy to older adults, evaluate needs, design plans of care, and document in the FOX EMR system.

Feel the autonomy to manage your own schedule, while collaborating with an interdisciplinary team and offering telehealth when appropriate to extend access and continuity of care.

Qualifications

• Valid Physical Therapy license in Michigan or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency; CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility and independence.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate.
