FOX Rehabilitation is seeking a licensed Physical Therapist for Morganville, NJ to deliver one-on-one in-home and senior living care. You will evaluate patients, design individualized plans, and provide evidence-based therapy to improve mobility and independence.

Qualifikationen

• Valid Physical Therapy license in state(s) of practice or eligibility to obtain
• Degree from an accredited Physical Therapy program (DPT preferred)
• Basic computer and EMR proficiency
• Current CPR certification

Aufgaben

• Provide one-on-one physical therapy to older adults in private residences or senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence and function
• Document effectively in FOX’s EMR per regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
