Job summary

FOX Rehabilitation is growing in Roswell, GA, and seeks licensed Physical Therapists to deliver one-on-one care to older adults in homes and Senior Living Communities. You’ll manage your own treatment schedule and may integrate telehealth for flexibility and continuity of care.

You will perform evaluations, create plans of care, and collaborate with an interdisciplinary team to improve mobility, safety, and independence for patients living in the community.

Qualifications

• Licensed Physical Therapist in the relevant state(s) or eligible to obtain license.
• Experience delivering one-on-one physical therapy in home or community settings is preferred.
• Comfort with telehealth and in-home care modalities.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and quality of life.
• Document in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
