Job summary

FOX Rehabilitation is seeking licensed Physical Therapists in Snellville, GA to deliver one-on-one geriatric care in homes and senior living communities. You’ll evaluate, plan, and provide evidence-based therapy, helping patients regain mobility and independence while coordinating with PT/OT/SLP and fitness specialists.

You’ll manage your own schedule with flexibility, including optional telehealth, and contribute to a patient-centered approach that emphasizes aging in place and strong

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences or senior living communities).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
