Job summary

Fox Rehabilitation is growing in Augusta, GA, and seeks licensed Physical Therapists to deliver one-on-one care to older adults in private residences and senior living communities.

You will develop individualized plans of care and deliver evidence-based treatments, with autonomy to manage your own schedule and patient relationships. Telehealth and in-home care options provide flexibility and broad reach.

Qualifications

• Licensed Physical Therapist in the state of practice or eligible for licensure
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification required

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy sessions supporting wellness, mobility, and functional independence
• Document accurately in FOX’s EMR system per regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
