Job summary

FOX Rehabilitation is growing in Edina, MN, and we’re looking for licensed Physical Therapists who want to deliver exceptional one-on-one care to older adults in their homes or within senior living communities. The model emphasizes autonomy, flexible scheduling, and meaningful patient outcomes, with optional telehealth to extend access.

Evaluations, individualized plans of care, and evidence-based treatment are core, with documentation in FOX’s EMR and collaboration with an interdisciplinary

Qualifications

• Valid Physical Therapy license in the state of practice or eligibility.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access and flexibility.
