Job summary

FOX Rehabilitation is growing in Glenn Mills, PA, and seeks licensed Physical Therapists to deliver one-on-one care to older adults in private homes or senior living communities.

You will conduct evaluations, develop plans of care, provide intensive therapy, document in FOX’s EMR, and collaborate with an interdisciplinary team. Optional telehealth components may be included to extend access and flexibility.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations and develop individualized plans of care.
• Deliver therapy sessions focused on wellness, mobility, independence.
• Document effectively in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth when appropriate to extend access.
