Job summary

FOX Rehabilitation is expanding access to geriatric physical therapy, delivering one-to-one care in patients’ homes and senior living communities across Edmonds, WA. Therapists enjoy autonomy in scheduling, build lasting patient relationships, and may integrate telehealth for continuity of care.

Licensed PTs provide evaluations, create plans of care, and collaborate with an interdisciplinary team to improve mobility, independence, and quality of life for older adults.

Qualifications

• Valid Physical Therapy license in WA or eligible to obtain
• Degree from an accredited Physical Therapy program
• Current CPR certification
• Basic computer and EMR proficiency

Responsibilities

• Provide one-on-one physical therapy to older adults at home or in senior living communities
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Document effectively within FOX's EMR system per regulatory standards
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and senior living staff to promote safety, function, and aging in place
• Provide telehealth/virtual care when appropriate to extend access and flexibility
