Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in Rochester, MN, in patients’ homes or senior living communities. You will design plans, provide evidence-based therapy, and document in FOX’s EMR while collaborating with PT/OT/SLP teams.

Flexibility to create your schedule, including telehealth options, supports autonomy and meaningful outcomes for older adults in the community setting.

Qualifications

• Licensed PT in Minnesota or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program.
• Current CPR certification required.

Responsibilities

• Provide one-on-one PT to older adults in home or senior living settings.
• Evaluate, plan and deliver evidence-based therapy.
• Deliver sessions focused on mobility, independence and function.
• Document in FOX EMR per regulatory standards.
• Collaborate with an interdisciplinary team (PT/OT/SLP/Fitness).
• Educate patients, caregivers and staff to promote aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
