Job summary

Fox Rehabilitation is expanding in White Bear Lake, MN and seeks licensed Physical Therapists to provide one-on-one geriatric care in patients' homes or senior living communities. You will evaluate, plan, and deliver evidence-based therapy while building meaningful patient relationships.

You’ll have scheduling autonomy, support from an interdisciplinary team, and optional telehealth to extend access and flexibility, improving mobility and quality of life for older adults.

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in their homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and aging in place.
• Document effectively within FOX’s EMR system according to regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and functional independence.
• Offer telehealth/virtual care when appropriate to extend access.
