Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one geriatric care in Palm Harbor, FL, delivering high-impact therapy in private residences and senior living communities.

You will evaluate, treat, and document patient progress, while enjoying autonomy over your schedule and opportunities to incorporate telehealth into care plans. Join a clinician-led culture focused on independence and aging in place.

Qualifications

• Must hold a valid Physical Therapist license in the state(s) of practice
• Degree from an accredited Physical Therapy program
• Basic computer literacy and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings
• Perform evaluations, develop plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence, and quality of life
• Document in FOX’s EMR system per regulatory standards
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
