Job summary

FOX Rehabilitation seeks licensed physical therapists to deliver one-on-one geriatric care in Palm Harbor, FL, including in-home visits and senior living settings. Clinicians can incorporate telehealth to extend access and flexibility while building strong patient relationships.

Qualifications

• Licensed Physical Therapist in the state(s) of practice or eligible to obtain licensure.
• Degree from an accredited Physical Therapy program (DPT preferred).
• Current CPR certification and basic computer/EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Develop and execute individualized plans of care with evidence-based treatments.
• Deliver therapy sessions to improve mobility and independence; document in EMR.
