FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one care to older adults in private residences and senior living communities in the Port St. Lucie area.

You will conduct evaluations, create individualized plans, and deliver evidence-based treatment, with the flexibility to balance in-home visits and telehealth while building lasting patient relationships. Qualifications

• Valid Physical Therapy license in the state(s) of practice or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsabilités

• Provide one-on-one physical therapy to older adults in home or community settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support mobility, independence, and functional goals.
• Document effectively in FOX’s EMR system.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth or virtual care when appropriate.
