Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to join our Lehighton, PA team providing one-on-one in-home and senior living care. You’ll evaluate, treat, and design individualized plans of care for older adults in their homes or communities.

This role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes, with the option to incorporate telehealth to extend care access and continuity.

Qualifications

• Licensed Physical Therapist in the state(s) of practice, or eligibility to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to support mobility, independence, and function
• Document in FOX’s EMR system in compliance with regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
