Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in Bound Brook, NJ, in private residences or Senior Living Communities through our Geriatric House Calls model. You’ll have autonomy to set your own treatment schedule, build lasting patient relationships, and optionally incorporate telehealth to expand access and continuity of care.

Required: NJ PT license, degree from an accredited PT program, EMR proficiency, CPR certification, and a passion for

Qualifications

• Licensed Physical Therapist in NJ or eligible to obtain license.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individual plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to improve mobility, independence and function.
• Document effectively in the EMR system per regulatory standards.
• Collaborate with an interdisciplinary team and educate patients and caregivers.
• Offer telehealth/virtual care when appropriate to extend access.
