Fox Rehabilitation in Smyrna, TN seeks a licensed Physical Therapist to deliver one-on-one geriatric care in patients' homes or senior living communities. Clinicians manage evaluations, plans of care, and evidence-based treatments, with telehealth options for flexibility.

Qualifications

• Licensed Physical Therapist in the state of practice (or eligible).
• Degree from an accredited Physical Therapy program.

Responsabilités

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence and function.
• Document within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs and Fitness Specialists.
• Educate patients, caregivers and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
