FOX Rehabilitation is seeking licensed Physical Therapists to provide in-home and senior living care in Smyrna, TN. You will evaluate patients, create individualized plans, and deliver evidence-based therapy to improve mobility and independence.

Clinicians can incorporate telehealth and set their own schedules while collaborating with an interdisciplinary team to support aging in place. A meaningful, patient-centered role awaits.

Qualifikationen

• Valid Physical Therapy license in the state of practice or eligible to obtain it.
• Current CPR certification.
• Basic computer and EMR proficiency.

Aufgaben

• Provide one-on-one physical therapy to older adults in home or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support mobility, independence, and functional goals.
• Document care in FOX’s EMR system compliant with regulations.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Offer telehealth/virtual care when appropriate to extend access.
