Job summary

FOX Rehabilitation in Smyrna, TN is hiring licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes and in senior living communities.

Qualifications

• Licensed Physical Therapist in Tennessee with current credentials.
• Experience in geriatric home health care preferred.
• Ability to work autonomously with flexible scheduling.

Responsibilities

• Provide one-on-one PT to older adults in homes or senior living.
• Perform evaluations and develop plans of care.
• Deliver therapy to improve mobility and independence.
• Document in FOX EMR according to standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers on safety and aging in place.
• Provide telehealth when appropriate.
