Job summary

Fox Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one, in-home physical therapy to older adults in Fredericksburg, VA. Therapists manage their own schedules, treat in private residences and senior living communities, and may incorporate telehealth when suitable.

You will collaborate with an interdisciplinary team and educate patients, caregivers, and staff to promote safety and aging in place while advancing your clinical autonomy and patient outcomes.

Qualifications

• Licensed Physical Therapist in the state(s) of practice, or eligible to obtain
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or in senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions to improve mobility, independence, and quality of life
• Document in electronic medical records per regulatory standards
• Collaborate with an interdisciplinary team (PT, OT, SLP, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
