Job summary
FOX Rehabilitation is seeking a licensed Physical Therapist to provide one-on-one geriatric home and senior living care in Warrenton, VA. You will evaluate, plan, and deliver evidence-based therapy for older adults.
Work autonomously with a flexible schedule, optional telehealth, and collaborative care from PT, OT, SLP, and Fitness Specialists to improve mobility and independence in the community. Join a clinician-driven culture that supports growth and professional fulfillment.
Qualifications
- Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
- Degree from an accredited Physical Therapy program.
- Basic computer and EMR proficiency.
- Current CPR certification.
Responsibilities
- Provide one-on-one physical therapy to older adults in homes or senior living communities.
- Perform evaluations, develop plans of care, and deliver evidence-based treatment.
- Deliver therapy sessions to improve mobility, independence, and safety.
- Document in FOX’s EMR in line with regulatory standards.
- Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
- Educate patients and caregivers to promote aging in place.
- Provide telehealth/virtual care when appropriate.
