Job summary

Fox Rehabilitation in West Hartford, CT is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or senior living communities. The Geriatric House Calls model emphasizes autonomy, flexible schedules, and meaningful outcomes, with some telehealth options to extend access.

Responsibilities include evaluations, plans of care, evidence-based treatment, EMR documentation, and collaboration with PTs, OTs, SLPs, and Fitness Specialists.

Qualifications

• Valid Physical Therapy license in the state(s) of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, longevity, mobility, and functional independence.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and senior living staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
