Job summary

Fox Rehabilitation in Granby, CT seeks licensed Physical Therapists to deliver one-on-one geriatric care in patients' homes or senior living communities. You will evaluate, plan, and tailor treatments to improve mobility and independence.

Clinicians may use telehealth and must document in an EMR, collaborating with PTs, OTs, SLPs, and other staff. Part- and full-time roles available with sponsorship options for qualified clinicians.

Qualifications

• Must hold a valid Physical Therapy license in CT or eligible to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer skills and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in home or community settings.
• Develop individualized plans of care and deliver evidence-based treatment.
• Deliver therapy to improve mobility, independence and aging in place.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with an interdisciplinary team.
• Educate patients, caregivers, and staff on safety and function.
• Offer telehealth/virtual care when appropriate.
