Job summary

Fox Rehabilitation is hiring Physical Therapists to provide in-home and senior living setting care in Greenwich, CT. The role offers autonomy, flexible scheduling, and meaningful patient outcomes through our Geriatric House Calls model.

Licensed PTs will evaluate, plan, and deliver evidence-based therapy, with opportunities to incorporate telehealth and collaborate with a multidisciplinary team to support mobility and aging in place for older adults.

Qualifications

• Valid Physical Therapy license in the state.
• DPT or equivalent degree from an accredited PT program.
• Basic computer familiarity and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in-home or in senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that improve mobility, independence, and quality of life.
• Document effectively within FOX’s EMR system in accordance with regulatory standards.
• Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
