Job summary

FOX Rehabilitation in Troy, MI is seeking licensed Physical Therapists to deliver one‑on‑one geriatric care in patients’ homes or senior living communities. You’ll evaluate, design, and deliver individualized plans of care that improve mobility, independence, and quality of life.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice (Michigan or eligible to obtain).
• Proficiency in EMR systems and documentation.
• Current CPR certification.

Responsibilities

• Provide one‑on‑one physical therapy to older adults in private residences or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence‑based treatment.
• Deliver therapy sessions to support wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate to extend access.
