Job summary

Fox Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in patients’ homes across Mantua, NJ and surrounding areas. The role emphasizes autonomy, flexible scheduling, and meaningful patient outcomes through a Geriatric House Calls model.

Clinicians may also incorporate telehealth into their schedules, treating in private residences or senior living communities with interdisciplinary collaboration and career growth opportunities.

Qualifications

• Licensed Physical Therapist in NJ or eligible to obtain license
• Degree from an accredited Physical Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one physical therapy to older adults in private residences or senior living settings
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
• Deliver therapy sessions that support wellness, mobility, and independence
• Document effectively in FOX’s EMR system in accordance with regulatory standards
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists)
• Educate patients, caregivers, and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate to extend access
