Job summary

FOX Rehabilitation is seeking a licensed Occupational Therapist in Lacey, WA to deliver one-on-one care in patients' homes and senior living communities. You will assess, plan, and implement evidence-based OT interventions, promoting mobility and independence with telehealth as needed.

Qualifications

• Valid OT license in WA or eligibility to obtain.
• Degree from an accredited OT program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one Occupational therapy to older adults in home or senior living settings.
• Perform evaluations, develop plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with an interdisciplinary team (PTs, OTs, SLPs, Fitness Specialists).
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth / virtual care when appropriate to extend access.
