Job summary

FOX Rehabilitation is expanding its in-home Occupational Therapy services in Greater Bradenton, FL, inviting licensed OT clinicians to join a one-on-one care model that supports independence and aging in place. As part of the Geriatric House Calls program, clinicians create their own schedules, may use telehealth, and collaborate with PTs, OTs, SLPs, and Fitness Specialists to deliver evidence-based care.

This role offers full-time, part-time, or PRN opportunities with competitive compensation

Qualifications

• OT license in state of practice or eligibility to obtain
• Degree from an accredited Occupational Therapy program
• Basic computer and EMR proficiency
• Current CPR certification

Responsibilities

• Provide one-on-one OT to older adults in home or community settings
• Perform evaluations and develop care plans with evidence-based treatment
• Deliver therapy sessions to improve mobility and independence
• Document in FOX EMR per regulatory standards
• Collaborate with PTs, OTs, SLPs and Fitness Specialists
• Educate patients, caregivers and staff to promote safety and aging in place
• Provide telehealth/virtual care when appropriate
