Job summary

Habitat Health seeks a Home Care Coordinator to assess participant needs and coordinate in-home care aligned with personalized plans. You will participate in the Interdisciplinary Team to ensure safe, effective, and person-centered home care delivery in clients' homes and communities.

The role requires clinical experience in home care or geriatrics, strong communication, and ability to manage multiple priorities in a fast-growing organization.

Qualifications

• Minimum of two years of clinical experience in home care, geriatrics, care coordination, or relatedHealthcare setting.
• Knowledge of care planning, clinical assessments, and home care service delivery.
• Ability to conduct comprehensive participant assessments and identify changes in condition or care needs.
• Strong clinical judgment and critical thinking skills.
• Excellent interpersonal, communication, and collaboration skills.
• Ability to work effectively within an interdisciplinary team environment.
• Strong organizational skills with the ability to manage multiple priorities and deadlines.
• Proficiency with electronic health records and healthcare documentation systems.
• Valid driver's license, reliable transportation, and active auto insurance as per state requirements.
• Demonstrates ability to use motivational interviewing and clear expectations to support homecare hours.

Responsibilities

• Exhibit Habitat’s values and participate as an active IDT member in care planning.
• Conduct comprehensive in-home assessments to evaluate care needs and safety risks.
• Perform home safety evaluations and provide recommendations to promote independence and reduce risk.
• Assess participants' ability to safely perform tasks and detect changes in condition.
• Collaborate with participants, caregivers, and family to identify care needs and service gaps.
• Coordinate home care services to align with individualized care plans and goals.
• Maintain current authorizations for services and ensure payer requirements are met.
• Review weekly notes to identify service gaps, changes in condition, and escalate concerns.
