Job summary

Habitat Health in California is seeking a Home Care Coordinator to ensure that personal and clinical homecare needs are delivered so participants thrive at home. As part of the Interdisciplinary Team (IDT), you will assess needs, develop care plans, coordinate services, and partner with caregivers to close gaps while maintaining safety and independence in the home setting.

Travel between sites may be required; you will document in EHRs, uphold safety procedures, and support quality improvement

Qualifications

• Minimum of two (2) years of clinical experience in home care, geriatrics, care coordination, or related healthcare setting.
• Knowledge of care planning, clinical assessments, and home care service delivery.
• Ability to conduct comprehensive participant assessments and identify changes in condition or care needs.
• Strong clinical judgment, critical thinking, and collaboration within an interdisciplinary team.
• Excellent interpersonal and communication skills; able to work with diverse stakeholders.

Responsibilities

• Conduct in-home assessments to evaluate participant care needs and safety risks.
• Develop care plans and coordinate home care services to meet individualized goals.
• Collaborate with participants, caregivers, and family to identify care gaps.
• Participate as a member of the IDT to implement and update care plans.
• Coordinate with clinical and non-clinical staff to ensure comprehensive care.
• Review weekly notes, service gaps, and SDRs; escalate issues as needed.
• Maintain accurate authorizations and ensure payer requirements are met.
• Ensure documentation in EHRs is timely, accurate, and compliant.
