Job summary

Cornerstones of Care seeks a Functional Family Therapy Provider to deliver FFT services to youth and families, primarily via in-home visits weekly. You will learn and apply the FFT model with ongoing FFT LLC consultations and collaborate with a clinical team under the FFT program.

The role requires a Bachelor’s degree (Master’s preferred), passing a background check, and a valid driver’s license. You will join a dedicated outpatient team guided by the Clinical Manager – FFT and support diverse

Qualifications

• Bachelor’s degree is required.
• Master’s degree preferred.
• Pass background check, physical, and drug screening.

Responsibilities

• Engage clients and families with a strength-based approach.
• Document services using the CSS system and plan treatment per FFT.
• Utilize MyAvatar to manage confidential client information.
• Provide FFT services to 10–12 families, mainly in-home weekly.
• Monitor progress and adjust treatment phases as needed.
• Maintain fidelity to the FFT model and phase system.
