This full-time, on-site role provides in-home clinical support to families with young children, focusing on safety, stability, and connection to community resources.

You will deliver case management, education, crisis support, and collaboration with a care team. A Bachelor's degree in human services and at least two years in a mental health setting are required;

Qualifications

• Bachelor's degree in human services required.
• At least two years' experience in a mental health setting preferred.
• Valid driver's license required.

Responsibilities

• Provides high-quality, client-centered services to children and families.
• Demonstrates competency in child development and trauma-informed care.
• Delivers case management, education, referral, and crisis support as needed.
• Collaborates with other providers to support client/family needs.
• Works with program staff during clinical crises.
• Assists families in connecting to community and natural supports.
• Participates in treatment and discharge planning.
• Completes all program/documentation per guidelines.
• Documents all client contacts in records.
• Supports families in connecting to appropriate level of care.
