Job summary

The Village in Hartford, CT is seeking an In Home Early Childhood Family Support Specialist to provide case management, mental health services, and family support in home or community settings. The role collaborates with DCF, schools, and community providers to stabilize children at home and transition to outpatient services.

Responsibilities include conducting assessments, developing treatment plans, ensuring timely documentation, and working with a team to deliver comprehensive care.

Qualifications

• BA in social work, sociology, psychology or comparable human service program.
• Associate's degree plus 2 years relevant experience can be accepted in lieu of bachelor's.

Responsibilities

• Provides direct services to children and their families and conducts comprehensive assessments.
• Develops treatment plans and contracts; implements and updates them as needed.
• Discharge planning and follow-up for continued care and community connections.
• Maintains clinical documentation and reports as required by policies and funding sources.
• Collaborates with schools, courts, hospitals and other community organizations.
