Job summary

Advocates is seeking a Family Support Counselor to provide home- and community-based services contracted through the Department of Children and Families. You will assess needs, coordinate treatment plans, and coach families to access resources and supports.

Qualifications

• Bachelor’s degree in social work, psychology, or related field, OR 5+ years of experience working with youth/families.
• Strong computer knowledge and ability to use an Electronic Health Record to document services.
• Ability to thrive in a fast-paced, team-oriented environment.
• Strong organizational skills and ability to multi-task.
• Outstanding reasoning and problem-solving abilities.
• Demonstrated understanding of and competence in serving diverse populations.
• Ability to read and write English.
• Must have valid driver’s license and access to insured automobile to transport members.

Responsibilities

• Conduct assessments of strengths and needs
• Complete crisis plans and coordinate treatment planning, in collaboration with family members
• Provide individual and family support and coaching
• Identify community resources and develop natural supports for youth and parent/guardian
• Provide comprehensive care coordination
