Job summary

Qualifications

• Bachelor’s degree in human services or education required.
• Associate degree with 2 years’ experience and pursuing a bachelor’s within 6 months.
• Valid driver’s license with good driving record.
• Ability to complete MDHHS and MFT documentation on time.
• Knowledge of the MFT philosophy and 2Gen framework.

Responsibilities

• Provide in-home services up to 180 days, with extensions to 270 days total.
• Maintain face-to-face contact with families per contract and needs.
• Complete MDHHS and MFT documentation within established timeframes.
• Engage families using Motivational Interviewing to build rapport and assess needs.
• Connect families with housing, transportation, employment, education resources.
• Create proactive safety plans and support Two-Generation goals.
• Attend court hearings and provide testimony when requested.
• Transport clients using personal vehicle as needed.
• Flexible scheduling to meet families’ needs.
• Collaborate with providers and case managers.
